The office staff will obtain some basic information, including your name, phone number, and email address. Additionally, we will obtain basic insurance information to perform a complimentary verification of your health insurance benefits. Finally, we will schedule your first appointment to meet with Dr. Berk.

After obtaining this information and scheduling your appointment, we will send you an email with a link to fill out our office's paperwork. Our office is striving to become paperless and efficient, so we ask that patients fill out paperwork prior to coming to the office.
